What aloe vera actually is, and why the form matters

Aloe vera is a succulent plant whose leaf contains two very different substances. The inner leaf holds a clear, soothing gel that's been used for digestive and skin support for thousands of years. Just beneath the outer rind sits a yellow sap called latex, which contains aloin, a compound with strong laxative properties and its own set of safety concerns when taken in unregulated amounts.

This distinction isn't a technicality. Products made from whole-leaf aloe, where the latex hasn't been properly removed, are a genuinely different substance from purified inner-leaf gel, with different effects and different safety profiles. Commercially sold oral aloe products are required to keep aloin content below 10 parts per million, and reputable inner-leaf products are independently certified by bodies like the IASC to confirm they meet that standard.

How aloe vera gel works in the gut

The inner gel is rich in polysaccharides, most notably acemannan, which has documented immune-modulating and anti-inflammatory properties. These compounds are thought to help form a soothing layer over irritated gastric tissue, working alongside the gel's glycoproteins and enzymes, which have their own anti-inflammatory activity independent of acemannan.

This gives aloe vera a slightly different role in a formula than pure mucilage botanicals like slippery elm or marshmallow root. What the research says

Aloe vera's clearest clinical data for gut health comes from gastritis and acid reflux research specifically, which matches where it's traditionally been used and where most people come to this ingredient looking for answers. A pilot randomised controlled trial of 79 people with GERD compared Aloe vera syrup against omeprazole and ranitidine over four weeks. All three groups saw reductions in the core GERD symptoms tracked, heartburn, regurgitation, belching, and related complaints, and the Aloe vera group had no adverse events requiring anyone to withdraw from the trial, compared with several in both medication groups.

That's a genuinely useful data point specific to gastritis and reflux symptoms, but it's one pilot trial, not a large body of evidence, and it isn't a reason to change or stop a prescribed medication on your own. What it does show is that inner-leaf aloe vera is a seriously studied ingredient for gastritis and reflux-type symptoms specifically, not just a vague wellness ingredient riding on a folk reputation. Separately, aloe's antibacterial properties have also shown some early evidence of supporting standard antibiotic treatment for H. pylori, the bacterium behind a large share of chronic gastritis cases, though this is early and not a substitute for the antibiotic course itself. Smaller research on IBS symptoms exists too, though gastritis and reflux are where the strongest and most specific evidence sits.

As with most traditional botanicals, the honest picture is a strong mechanistic case and consistent traditional use, alongside a research base that's still growing. That's not unique to aloe vera, it's the position most plant-based ingredients are in, because large trials are expensive and tend to follow commercial funding rather than traditional use. It's a reason to hold reasonable expectations, not a reason to dismiss the mechanism.

Why inner leaf and purity aren't optional

Some of the more alarming aloe vera headlines, animal studies linking whole-leaf extract to cancer risk, come specifically from non-decolorized whole-leaf products where the latex and aloin haven't been removed.
