Aloe vera is a succulent plant whose leaf contains two very different substances. The inner leaf holds a clear, soothing gel that's been used for digestive and skin support for thousands of years. Just beneath the outer rind sits a yellow sap called latex, which contains aloin, a compound with strong laxative properties and its own set of safety concerns when taken in unregulated amounts.

How aloe vera gel works in the gut
The inner gel is rich in polysaccharides, most notably acemannan, which has documented immune-modulating and anti-inflammatory properties. That's a genuinely useful data point specific to gastritis and reflux symptoms, but it's one pilot trial, not a large body of evidence, and it isn't a reason to change or stop a prescribed medication on your own. What it does show is that inner-leaf aloe vera is a seriously studied ingredient for gastritis and reflux-type symptoms specifically, not just a vague wellness ingredient riding on a folk reputation. Separately, aloe's antibacterial properties have also shown some early evidence of supporting standard antibiotic treatment for H. pylori, the bacterium behind a large share of chronic gastritis cases, though this is early and not a substitute for the antibiotic course itself. Smaller research on IBS symptoms exists too, though gastritis and reflux are where the strongest and most specific evidence sits.
As with most traditional botanicals, the honest picture is a strong mechanistic case and consistent traditional use, alongside a research base that's still growing. That's not unique to aloe vera, it's the position most plant-based ingredients are in, because large trials are expensive and tend to follow commercial funding rather than traditional use.
